Texas has the highest share of people without health insurance in the country, according to the U.S. Census Bureau. You can feel it in Houston: plenty of people work, pay their bills and still have no coverage. Being uninsured doesn't mean going without care; it means choosing well where to go for what you need.

Where do I go, depending on what's wrong?

  • A real emergency (chest pain, trouble breathing, signs of a stroke, bleeding that won't stop): go to the emergency room or call 911, insured or not. A federal law (EMTALA) requires hospital emergency rooms to assess and stabilize you whether or not you can pay.
  • Everyday problems (infections, fever, sore throat, cuts, blood pressure or blood sugar checks, lab work, work or school exams): a walk-in clinic with per-service pricing is usually the fastest and most affordable option.
  • Long-term follow-up with financial help: community health centers and county programs.

What programs are there in Houston?

  • Community health centers. They charge based on your income. Find the nearest one with the HRSA locator.
  • Harris Health financial assistance. Harris County's health system has an assistance program for residents who meet certain income and residency requirements.
  • The insurance Marketplace. If your situation allows it, HealthCare.gov shows whether you qualify for a subsidized plan.

These programs involve paperwork and sometimes a waiting list, so it helps to pair them with a clinic you can walk into on the day you need it.

How do I avoid surprises on the bill?

  1. Ask for the price first: for the visit, each lab test and each procedure.
  2. Ask what is needed today and what can wait.
  3. Bring your medications and previous results so tests aren't repeated.
  4. Use current promotions on checkups and lab panels, if they fit what you need.

What about medications?

They are the other half of the bill. Always ask whether a generic version exists: it has the same active ingredient and usually costs much less. If you take something every day, ask what each pill is for and how long you need it; treatments that piled up over the years can sometimes be simplified. And don't stop a medication because of the price without telling your doctor: there are cheaper alternatives that don't put your health at risk.

What about chronic conditions?

Diabetes, high blood pressure and cholesterol are exactly the conditions most often neglected without insurance, and the most expensive when they get out of hand. Regular follow-up, with lab work every few months and treatment adjustments, costs far less than a hospital stay. If you already have one of them, read our guide to managing diabetes.
