School & Sports Physical Exams

Before enrolling in school, starting daycare or playing a sport, children and teens need a physical exam. At Clínica Hispana Nueva Salud we do it quickly and thoroughly, with all forms ready: no appointment, no insurance required and in Spanish.

What's included?

• Head-to-toe general physical exam
• Vital signs: weight, height, blood pressure, heart rate
• Vision and hearing screening
• Heart, lung, abdomen, spine and joint check, important for sports
• Review of vaccination history, allergies and prior conditions (asthma, etc.)
• Completion and signature of forms from the school, district (HISD, Pasadena ISD and others) or sports league
• Health recommendations for the family, in Spanish

What is it for?

• School enrollment (kindergarten, elementary, middle, high school) and daycare
• Sports physical for soccer, baseball, basketball, track, cheer and any school activity
• Summer camps and extracurricular programs
• Annual checkup to make sure your child is growing healthy

If your child needs a TB test or a vaccine booster for the same requirement, we do it during the same visit.

What should I bring?

• The school or team form (if you have it; otherwise we use the standard format)
• The child's vaccination record
• A list of medications they take and known allergies
• Glasses or hearing aids, if used
• A parent or guardian must accompany minors

When should I do it?

Ideally 2 to 4 weeks before school or the sports season starts, to allow time for any additional test or vaccine. Demand rises in July and August; coming early avoids waits.

What is the visit like?

1. Walk in without an appointment, check in and hand over the form and vaccination record.
2. Vital signs, weight, height, vision and hearing are taken.
3. The medical staff performs the full physical exam and reviews the history.
4. The form is completed and signed; you leave with everything ready to turn in at school.
