Immigration Medical Exam I-693

If you are adjusting status to become a permanent resident, USCIS will ask for Form I-693 with the results of your medical exam. At Clínica Hispana Nueva Salud we perform it with a USCIS-authorized physician (civil surgeon), with sample collection and vaccines at the same clinic and everything explained in Spanish.

What does the service include?

• A review of your medical history and vaccination record.
• A physical exam plus mental health and substance use evaluation, as the CDC requires.
• A tuberculosis blood test (IGRA) and the lab tests that apply to your age.
• Any vaccines you are missing.
• Completion and signature of the I-693 by the authorized physician once all results are in.

How can I confirm the doctor is authorized?

USCIS's official Find a Civil Surgeon tool lets you search by ZIP code. We recommend doing this with any clinic before you pay.

What should I bring?

A photo ID, your vaccination record (from here or your home country), a list of your medications and, if you were ever treated for tuberculosis, those documents. Don't sign the form before you come in.

How long does the whole process take?

The clinic visit is a single one, but the form is completed once the lab results arrive. If a test comes back abnormal, an additional study may be needed before it can be finished. You'll hear from us the moment it's done.
