Diabetes, Hypertension & Cholesterol Care

Diabetes, high blood pressure and high cholesterol have one thing in common: they barely hurt while they do damage. That's why management isn't about coming in when you feel bad, but about keeping your numbers in range month after month. At the clinic we handle that follow-up with you, walk-in and in Spanish.

What happens at each follow-up visit?

• We check your blood pressure, weight and, when needed, blood sugar in the office.
• We review your lab work: hemoglobin A1c, glucose, lipid panel and kidney tests.
• We go over your medications, how you take them and any side effects.
• We adjust treatment and set the date for your next check.

What are the targets?

Everyone has their own, but as a reference: many adults with diabetes aim for an A1c under 7% (American Diabetes Association), and for most patients with hypertension the goal is under 130/80. The medical team tailors the target to your age and other conditions.

Does checking my blood pressure at home help?

A lot. Home readings show your everyday blood pressure without the stress of the office. Sit for five minutes first, back supported, feet on the floor and arm at heart level, and use an upper-arm cuff rather than a wrist one (American Heart Association). Bring your log to every visit.

What else needs checking?

• Kidneys: with blood and urine tests, at least once a year if you have diabetes or high blood pressure.
• Eyes: a yearly dilated eye exam with an eye doctor if you have diabetes.
• Feet: check them daily at home and at every visit, looking for sores or numb spots.

What if I don't have insurance?
